If you have Medicaid, a medicaid managed plan, HMO, EPO or POS plan, ensure Dr. Faweya (NPI #: 1609104785) is listed as your child's Primary Care Physician (PCP) with your health insurance provider. Be sure to ask for a reference number when you make the change as you will need it when scheduling an appointment. Please include this reference number in your meeting notes.
You can change your primary care provider (PCP) online through your insurance provider's website – however this change is usually effective on the first day of the following month. If your desired appointment is sooner, call your insurance provider's customer service line or personal health guide for assistance in changing your PCP. Be sure to ask for a reference number, you will need it before you can schedule an appointment.
